Description of the Organization Harris County Hospital District (HCHD) Health Care for the Homeless Program (HCHP) has a vision to maintain a leadership position in the provision of access to healthcare services for homeless individuals. Homeless individuals encounter monumental barriers as they attempt to access exisitng health services because of financial, cultural, psychosocial, and/or physical barriers. Using the mobile health program, the HCHD encourages and enables homeless individuals to access services that they so desperately need. Mission The Health Care for the Homeless Program exists to improve the health status of the homeless individuals of Houston and Harris County by: - Encouraging and enabling access to new and existing health care, mental health, and substance abuse facilities.
- Enhancing communication and cooperation among agencies that serve homeless individuals.
